Upon completing this form, you will be required to pay the $25 application fee by credit card. 

DO NOT use this application if you have previously paid the $25 undergraduate application fee at MTSU even if you did not attend classes.  You are not required to pay the application fee again. You will need to use the printable application to avoid paying the application fee for a second time. 

DO NOT use this application if you have previously attended classes at MTSU.  If you have attended classes at MTSU before, and have sat out at least one semester, not including summer, go back and complete the re-enrollment application.
