Special Instructions
- State, MTSU, LGI, TBR and UT employees and Permanently Disabled and Senior Adult students using the fee waiver program cannot preregister for the course(s). Please refer to the "Dates at a Glance" page in the Registration Guides for fee waiver registration dates.
- Fee Waivers are accepted for one (1) course per part-of-term (up to four credit hours) for a maximum of four courses per academic year (fall/spring/summer). An employee may waive two (2) courses during the term as long as the course sessions (instruction periods) do not overlap. A separate form is required for each semester and summer course and must be submitted to the Business Office before the fee payment deadlines.
- Discounts and waivers cannot be used in conjuction with any other type of fee waiver or discount program.
- A separate form must be submitted each semester to verify continued employment/eligibility and the employer certification signature should be obtained no earlier than 30 days before the fee payment deadline for the semester. Signatures may be obtained and submitted beginning July 1 for the fall semester, November 1 for spring, and April 1 for summer.
- Teacher discount forms should be certified and display school or superintendent seal or stamp.

You may fax the completed form to (615)904-8169 or email bursarmt@mtsu.edu. Please do not include social security numbers if you chose to fax or email a copy of the form. Please use the student's MTSU ID number. If you have any questions, please email bursarmt@mtsu.edu.
MTSU will not accept any discount or waiver form after the last day of final exams for the semester. Discounts and waivers are not retroactive.
